Ischemic arterial stroke, epileptic status and choreoathetosis in late vasculitis COVID-19: a case report

Case presentation: A 3-year-old patient started with runny nose and fever onset treatment for pneumonia, without improvement amoxacillin Clavulanic Acid 10 days; later azithromycin 5 days, joined our service due to impaired respiratory function, when performing chest computer tomography (CT): seen opacities in matte glass bilaterally. Screening tests COVID-19 the initial care unit were negative. The evolved pleural effusion, convulsive status, left complete hemiplegia. Due worsening breath was intubated, cranial-CT showed multiple infarctions, compromising bilateral left-wing of middle cerebral artery (MCA) territory, associated diffuse brain edema, cranial CT angiography: occlusion proximal segment cervical top intracranial right internal carotid (ICA), MCA anterior (ACA) A2 segment. There wasn’t no history trauma. We performed a study vascular wall by MRI (“black blood”) that parietal thickening thrombosed segments, as well foci concentric enhancement, representing inflammatory process. After extubation, she developed paroxysmal autonomic instability, dystonia; and, but later, choreothetosis side. Performed viral panel liquor including research COVID-19: negative; serology this virus IgG IgM positive. Rare causes stroke children negative investigations. During evolution, anticoagulation performed, achieved adequate control seizures, currently deformity prevention motor rehabilitation.
